Who can have NHS continuing healthcare?

You must be assessed by a team of healthcare professionals as having a ‘primary health need’ for care. This assessment will normally start by a checklist that indicates if you should have a full assessment.

If you’re referred for a full assessment, the relevant health and social care professionals will work together to get a full picture of your physical, mental health and social care needs. It is not decided automatically because of any particular diagnosis or condition. They will consider things like:

  • What type of needs you have
  • How severe and complex they are
  • How unpredictable your needs can be
  • Any risks to you and others if adequate and timely care is not provided
