Lynda and Gwen’s story
Lynda contacted Prevention Matters after hearing about the service from Carers Bucks. She had gone to them for some advice on caring for her mum who is in her nineties and lives with Lynda and her family.
Gwen lives with her daughter Lynda and her family. Gwen has a level of dementia and a visual impairment. Lynda was looking for activities for her mum to give her more outside stimulus and also so that Lynda could have a break.
CPW: Jackie
Jackie, the Community Practice Worker, visited Lynda and Gwen and the following outcomes were agreed:
- Information on the carers grant for the daughter to apply for.
- A referral was made to BCC asking for advice and guidance regarding respite care and the carer’s break fund for when the daughter goes away on holiday.
- A referral was made to Bucks Sensory Service for an assessment and advice. The rehabilitation officer has visited and provided specialist equipment for Gwen and given advice on the talking book service.
- Information on singing for the brain and dementia café held in the church hall close by. These will be social events that both Gwen and Lynda can go along to together.
- They also talked about day centres so Jackie made a referral and Gwen is now attending regularly and will probably go on two days a week in the future.
Outcomes:
- Access to carer’s break fund while Mum goes on holiday
- Referral to Bucks Sensory Service and specialist equipment provided and advice on talking book service
- Singing for the Brain and Dementia Cafes
- Attending a day centre two days a week
