The Care Programme Approach

If the person you care for is experiencing severe mental health problems they would be referred for an assessment by a mental healthcare professional.

For some this will result in an allocation to a community mental health team and the care and support required delivered to them by a system called the Care Programme Approach

A care co-ordinator will assist in developing a care plan that covers both health and social care needs and it will be regularly reviewed with them by that professional. With the patient’s permission anyone who provides care and support can be included in this process.

If someone you care for has their mental health needs assessed the assessment should identify you as their carer. You will be told about your own right to a carer’s assessment and support.
