How we do it
- Provide support to all staff, including Carers, Nurses and other relevant member of staff by delivering both clinical and non-clinical workshops.
- Provide bespoke Management support when required.
- Prevention - promotion of the positives of safeguarding, Supporting action planning, risk assessments and positive risk taking.
- Support Care Providers in finding sustainable solutions to meet contractual requirements.
- Deliver Study days, to support the skills / knowledge development including competencies of nurses.
- Advise on Care and referring to the most appropriate resource.
- Support health promotion initiatives including wellbeing, nutrition, hydration and skin integrity amongst others for staff and residents (including informal carers, families and residents where applicable)
- Liaise with GP’s, pharmacists and other agencies to promote and enable care homes to provide the best possible person-centred care.
Who do we support?
We support all Services providing care for Adults across Buckinghamshire. These include Residential Care Homes, Nursing Homes, Providers supporting clients with Learning Disabilities and Domiciliary Care providers.
Who do we work with?
We work in partnerships with Safeguarding, Care Management, Contracts, Community Nursing, Specialist Nursing and Therapy Services across Health and Social Care.
We also work with Key Partners such as Care Quality Commission, Bucks Healthcare NHS Trust, Buckinghamshire County Council, Oxford Health Foundation Trust, Clinical Commissioning Group and national companies when relevant.
