If you have to go into hospital, we can help you get back home afterwards to regain your health, confidence and independence.

We have hospital social care teams working in NHS hospitals who will talk to you about the help you will need when you go home. Depending on your circumstances this could be:

  • Reablement homecare: help in your home for up to six weeks.
  • Equipment in your home to help you with washing, dressing and getting to the toilet.
  • A range of other services including meals delivery

While you are in hospital, ask the ward staff to make sure that the hospital social care team comes to talk to you about the help you will need, and to help make the arrangements.

Continuing healthcare: if you have an ongoing serious health condition and are registered with a GP in Buckinghamshire, the NHS may provide services in your home, or in a care home. You can talk about continuing health care with staff on the ward or the hospital social work team.
