After your assessment
If we are able to help we will draw up a support plan with you.
You will receive a copy of your assessment and support plan. If you had an assessment with the person you care for there may be a shared support plan, if you had a separate carer assessment, you will have your own plan.
You may be given the option for a support broker to help you (and the person you care for) to develop a support plan using direct payments.
You will be given information about meeting again in the future to review your circumstances and information on how to get in touch if your circumstances change.
